Replacement Unit Form:

If you have contacted customer service and you have been approved to receive a replacement unit, please complete the following form:Replacement Unit Form.

1. Print out two copies of the attached service form, one for yourself and one to send with the unit.
2. Indicate the model and serial number of the unit.
3. On the service form, please describe the issue that you are having.
4. On the shipping label, please put ATTN TO: SERVICE DEPARTMENT.
